If you are purchasing prescription eyewear from us, you may also be asked to provide some details of; Date of Birth, National Insurance Number, NHS number, Doctors Surgery, Health and Ocular Health history/ symptoms, Spectacle or Contact Lens prescriptions and Next of Kin. Some or all of this information may be necessary in order to fulfil a professional, safe and efficient service to you.
